The DICER1 familial cancer syndrome: clinical and molecular update

چکیده

DICER1 is an RNase endonuclease important for production of microRNAs, which regulate multiple proteincoding genes involved in growth and development. It has been linked to several tumors including pleuropulmonary blastoma (PPB), cystic nephroma, ovarian Sertoli-Leydig cell tumors and thyroid neoplasia. Most of the manifestations of DICER1 mutations occur in young children, adolescents and young adults; only thyroid neoplasia are diagnosed throughout adulthood. Although mutations in this gene behave as autosomal dominant tumor suppressor genes, the penetrance of the various tumors is highly variable, rendering screening and genetic counseling challenging. We have recently hypothesized that mutations in this gene are also responsible for the very rare but often fatal cases of infant (< 3y) Cushing Disease, which can be missed initially because of absent or only minimal signs of intracranial hypertension and hypercortisolemia in this age group. Eye signs, such as strabismus or proptosis, may be the first manifestations of these tumors. The ACTH-producing pituitary tumors seen in these infants have a histopathology and electron microscopy that has been termed pituitary blastoma. The incomplete penetrance of this and the other manifestations of the syndrome makes the involvement of additional predisposing factors highly likely, candidates of which are under investigation. This presentation will highlight the cardinal features of this new familial cancer syndrome, review the molecular biology of DICER1, consider additional polygenic influences on tumor development and discuss the current recommendations for following these families.

Germline mutations of DICER1 in Chinese women with BRCA1/BRCA2-negative familial breast cancer.

Germline mutations in identified breast cancer susceptibility genes account for less than 20% of Chinese familial breast cancers. Dicer is an essential component of the microRNA-producing machinery; germline mutations of DICER1 have been confirmed in familial pleuropulmonary blastoma, ovarian sex cord-stromal tumors, and other cancers. Low expression of DICER1 is frequently detected in breast c...

DICER1 syndrome: clarifying the diagnosis, clinical features and management implications of a pleiotropic tumour predisposition syndrome.

BACKGROUND Constitutional DICER1 mutations were recently reported to cause familial pleuropulmonary blastoma (PPB). AIM To investigate the contribution and phenotypic spectrum of constitutional and somatic DICER1 mutations to cancer. METHODS AND RESULTS The authors sequenced DICER1 in constitutional DNA from 823 unrelated patients with a variety of tumours and in 781 cancer cell lines. Cons...

Association of Pathogenic Missense and Nonsense Mutations in Mitochondrial COII Gene with Familial Adenomatous Polyposis (FAP)

Nuclear genetic mutations have been extensively investigated in solid tumors. However, the role of the mitochondrial genome remains uncertain. Since the metabolism of solid tumors is associated with aerobic glycolysis and high lactate production, tumors may have mitochondrial dysfunctions. Familial adenomatous polyposis (FAP) is a rare form‌ of colorectal cancer and an autosomal dominant inheri...

DICER1 mutations in familial multinodular goiter with and without ovarian Sertoli-Leydig cell tumors.

CONTEXT Nontoxic multinodular goiter (MNG) is frequently observed in the general population, but little is known about the underlying genetic susceptibility to this disease. Familial cases of MNG have been reported, and published reports describe 5 families that also contain at least 1 individual with a Sertoli-Leydig cell tumor of the ovary (SLCT). Germline mutations in DICER1, a gene that cod...

Predisposition to pediatric and hematologic cancers: a moving target.

Our understanding of hereditary cancer syndromes in children, adolescents, and young adults continues to grow. In addition, we now recognize the wide variation in tumor spectrum found within each specific cancer predisposition syndrome including the risk for hematologic malignancies. An increased understanding of the genetic mutations, biologic consequences, tumor risk, and clinical management ...
